Selected retirement age form

Please complete and submit this form if you are a member of either the Money Purchase 2003 Section, Auto Enrolement Section or have Additional Voluntary Contributions and wish to select a retirement age other than the normal retirement age of 62. By changing your retirement age, your investment funds may change if you are in one of the lifecycle options due to the automatic fund switching process.

Please note that any change to your selected retirement age will be applied to all your records with Aviva that are in the Plan.
